Our employees are focused on meeting the needs and exceeding the expectations of our patients and families by providing dignified, compassionate, and individualized care to each patient that we have the privilege to serve. Parks Healthcare and Rehabilitation Center is part of our “Journey Home” program, a significant percentage of our patients return home following a brief rehabilitative stay with us. For residents for whom living independently at home is no longer possible, we offer long-term care services in a comfortable and home-like setting. Find a location near you and speak with our knowledgeable staff about options that are right for you or your loved one. Consulate Health Care centers offer post-acute services for those patients who no longer require an in-patient hospital stay but who are not ready to go home. Our outcome-oriented rehabilitation therapy programs help patients rebuild the strength, function and mobility needed for a successful return home. Comprehensive rehabilitation services are provided in-house at every Consulate Health Care center.Rehabilitation services include Physical Therapy, Occupational Therapy and Speech Therapy. Our rehabilitation professionals are licensed therapists who are specially trained to meet the complex needs of patients recovering from a wide range of illnesses or injuries.
