Hospice Care Center – Port Orange

Located on Dunlawton Avenue in Volusia County., this 16-bed care center features a memorial walkway around the building with a gazebo for patients and families to enjoy. Each patient room features its own patio, bathroom, individualized climate control, private telephone, internet access and television. There is a great room for families to sit in with a piano and children's corner, as well as a Chapel available for patients and family use. Our goal is to provide patients and their families with expert and compassionate end-of-life care. Halifax Health - Hospice is dedicated to providing extraordinary, compassionate and professional end-of-life care. Our approach to care is simple. We provide expert and compassionate care to patients and families, while honoring their wishes and assisting in celebrating their lives one day at a time.  We improve the quality of the lives of our patients and their loved ones by reducing the anxiety, pain and burdens that may accompany illness. At Halifax Health - Hospice, we have a collaborative approach to care. Every day, our uniquely specialized team of physicians, registered nurses, social workers, spiritual care counselors, aides, bereavement counselors, complementary therapists, and volunteers work collectively to provide exceptional care for patients and families. We believe a person’s life should be cherished and celebrated. We have cared for patients in Volusia and Flagler counties for over 35 years and have become the established not-for-profit leader in providing hospice services in this community.
