Delaney Park Health & Rehabilitation Center has been serving Orlando, Orange, Osceola, Lake and Seminole counties for over 40 years. Our facility is located close to local hospitals. We provide post-acute services, rehabilitative services, skilled nursing, short and long term care through Physical, Occupational, and Speech Therapists; Registered and Licensed Practical Nurses; and Certified Nursing Assistants. This is complemented by Social Services, Activities, Nutritional Services, Housekeeping and Laundry Services. With over 80 years of combined experience, our staff of professionals is here to meet the needs of each and every patient and resident. Our post-acute services assist those transitioning from hospital to home. Our team of professionals will work with the physician to provide the level of care and services that will help each individual meet their goals for a successful discharge home. For those residents who can no longer be on their own and need long term care we offer general nursing and restorative services in a warm and home like environment. We provide a full-range of healthcare services to meet the changing needs of those we serve. Whether you or someone you care for is recovering from surgery or a serious illness, or has decisions regarding short or long-term care that need to be made, we understand how complex matching your needs with a health care provider can be. We design our individual plans around your personal healthcare needs. Rehabilitation services are included in the plan of care for stabilized patients who would benefit from a combination of physical, occupational or speech therapies. Our comprehensive rehabilitation program is customized to meet the changing needs of each patient as they progress through recovery. The goal of our rehabilitation program is to increase the level of independent functioning of the patient. The facility is staffed with licensed nurses 24 hours per day and a team of licensed Physical, Occupational, and Speech therapists. The interdisciplinary team works with the attending physician to develop an aggressive rehabilitation program and ensure the complex medical needs of the patients are addressed and a comprehensive plan of care is developed to help these patients return to their normal level of functioning as quickly as possible.
